Acute Watershed Infarction Following Autologous Fat Injection for Soft Tissue Augmentation of the Face

Abstract
The reports of fat embolism after autologous fat injection have increased as plastic surgery has become more popular and accessible. The neurological symptoms of cerebral fat embolism are variable, and the clinical diagnosis is difficult. A 32-year-old woman with no history of cardiovascular risk factors presented to our emergency room suffering from an acute change in mental state within 35 min of a fat infusion into her nasal bridge. Brain magnetic resonance imaging showed a left anterior and posterior watershed infarction. A funduscopic examination revealed a cherry red spot in the macula, and she was diagnosed with central retinal artery occlusion of the left eye. Based on our case, it is likely that the force and velocity applied during fat injections play a major role in the development of ocular and cerebral fat emboli. Thus, injections of fat into facial tissue should be performed slowly with minimal force for preventing unexpected complication.
